Cancellation must be made in writing or by the completion of this form.
Without the completion of this form memberships cannot be cancelled.
The two week cancellation notice begins from the date the form is completed and lodged with Bould Move. If we have not received it then we cannot know to cancel your membership.
If you did not receive a confirmation of the completion of the form it did not submit so please go back and submit it again.
While we collect your identifying information, this is for the membership team to cancel the correct member and membership. The feedback questions are anonymous and are used on an overall assessment of members experience and not individually.
